We offer a Membership Program called our Oral Wellness Program

For a one time annual fee you can activate a membership that is valid from 1 year from the date of purchase. Since this is an in office program there is no activation processing time. Program is active the moment package is purchased.

What is included at NO additional cost:

• At least 2 general cleanings (depends on package purchased)

• Yearly xrays
• Yearly photos
• Yearly exam
• PLUS discounts on treatment needed offered up to 50% discount
• PLUS a free gift

Family packages purchased together or within 30 days of first package is offered at a discount

Rules of program: Included services not rendered are non transferable, not refundable, and cannot roll over to the next contract year. It is the patients responsibility to schedule within the contract year to complete included services. There are no frequency rules that apply. There are no maximum limitations to discounts applied to services rendered.
